Write it in STAR, 90–120 seconds. What you do not say: 'He was lazy', 'Nobody listens to ops', 'I went to HR immediately' unless you actually did and it is the story. You are showing you can disagree about a process without making the floor the villain. Names stay out. Titles stay in.
